The Five Star chart and instructions are confusing, however ignore the first jacket size column and look at the second ‘Suitable for Chest’ column, which is in red.
For example: If your chest measures 43-44 inches the chest size of the jacket is 48 ins and this will provide the 4-5 ins of space, which is also known as ‘ease’, to allow for a decent fit.

Of course, this all depends on how accurate Five Star actually are in making their jackets and whether they can work to all the exact measurements they state in their charts.

So if im looking in the red column, does that take into consideration my chest size when layered?

For example my chest size is 43 naked and 46 with a sweater. Should I order suitable for 46-47 which is a Large?
