I'm 6'1 200 and wear 42l and 44l. 42 is a very trim fit . I think 44 would easily work for you unless you want a very trim fit
Hospitals are always cold. People wear wind breakers indoor all the time. In fact the hospital gives out logo ones for bonuses. So instead of a white coat I wear a black one. Joey I'm sure was just cold
Well I take it off when I scrub, but I leave it in the monitoring area. And I wear it to round. Patients who can't pronounce my name ask for the doctor in the leather jacket!!

I have an RMC 30's Nelson size 42. My shoulders and chest are the same as yours although my arms are longer. The 42 is slim, it looks great but is a little constraining on movement. I think its really a matter of preference, but I would think a 44 would be a standard fit and a 42 a fitted...
