I should have a DB odd waistcoat coming in soon. My main concern is whether or not it will be too much fabric that it cause problems, so I may wait until they arrive and then decide.
It was very common in the 30's. All my vintage 30's DB suits have (single-breasted) waistcoats.
I am also thinking I could order DB waistcoats anyway, and just wear them as odd waistcoats if I think it's too much. Hmm...
I have two double-breasted suits, and I've decided I want to get two matching waistcoats ordered for them in the same fabric.
At the same time I've been longing for some double-breasted waistcoats. I love how they look in a three-piece, but most of the time, I see them with single-breasted...

I'd throw the color in with a burgundy tie and nice pocket square rather than worry so much about the shoes. A good dark burgundy cap toe would work well too, though, I think.
Stick to a 40S. If you *actual* chest measurement is 40", a 38 will be too small. Look for 18 - 18.5" shoulders instead (just a guess... have someone measure you're own shoulders if you can.) If a 40S still feels roomy, it's generally easier to take things in/shorten them than to take them...

Being 5' 6" myself, I take it from the Duke and wear whatever I want. Just feel comfortable and appear confident.
2-button vs. 3-button or SB vs. DB doesn't really make much of a difference. I wear all four proudly. With cuffs even. As always, just make sure it fits.
