It came with a plain brown box and I found a mailing label and postmark on the bottom of the box. The addressee's name has the same intials as are stamped in the hat, so I'm assuming the two are related. The postmark is dated January 26, 1959.

Any info about the model would be appreciated,

The box seems linked to the person who owned the Borsalino, but not sure that the box and postal ticket are connected to this Borsalino.
